New resource to help people add their care wishes to the Summary Care Record

Age UK, our partners in the Health and Wellbeing Alliance, and the Future Planning project have produced a new leaflet, Future Planning: your wishes.

The leaflet aims to encourage people to discuss and write down their wishes about their care, information about health conditions and how they would like these to be managed in emergency situations, and to talk to their GP about including this information in the national Summary Care Record.

The resource has been produced at the request of ambulance service paramedics who attend people at times of crisis and often without up to date clinical information.
