Carers In Transition: Opportunity to Input to Project for Department of Health and Social Care
FaithAction are working on a project led by the Carers Trust for the Department of Health and Social Care about carers in transition – looking at how carers can be best supported to plan for when they are able to provide less or no care. This may be due to the age of the carer or their own physical/health needs. The end result will be the creation of a toolkit by Carer’s UK and range of resources for service providers to enable them to give the them the tools and knowledge to support carers through the transition period.
FaithAction is looking into the additional challenges faced by BAME communities in particular and we looking for existing case studies and reports that demonstrate how much support carers receive in planning for the transition period, and how accessible this support is.
