Carers In Transition: Survey
FaithAction are working on a project led by the Carers Trust for the Department of Health and Social Care (DHSC) about carers in transition – looking at how carers can be best supported to plan for when they are able to provide less or no care. This may be due to the age of the carer or their own physical/health needs. The end result will be the creation of a toolkit by Carer’s UK and a range of resources for service providers to enable them to give the them the tools and knowledge to support carers through the transition period.
